About the Role

We are seeking a skilled and experienced Nurse Coordinator to join our Continuing Healthcare team at NHS Herefordshire and Worcestershire Integrated Care Board.

Key Responsibilities
  • Conduct comprehensive assessments to determine eligibility for NHS Continuing Healthcare
  • Provide case management services, including reviews for eligible clients for an appropriate package of care or placement
  • Chair complex multidisciplinary team meetings
  • Develop and maintain knowledge of the NHS Continuing Healthcare National Framework and relevant legislation
Requirements
  • Registered Nurse with the NMC
  • 3 years post-registration experience
  • Driving license and access to a car
  • IT literate
  • High standard of English literacy skills, written and verbal

Person Specification
  • First-level qualification RGN, RMN, or RNLD Registration
  • Relevant degree or extensive knowledge, skills, and experience to equivalent level
  • Evidence of continuing professional development
  • Significant post-registration experience
