We are seeking a skilled and experienced Nurse Coordinator to join our Continuing Healthcare team. The successful candidate will be responsible for coordinating and chairing CHC assessment meetings, package of care reviews, and case management in relation to NHS commissioned care.
About the RoleThis is a community-based position, requiring the ability to work independently to deadlines, with excellent communication skills to engage patients and/or their representatives. The successful candidate will need to be proficient in the use of Microsoft Word and have confidence in leading meetings both face-to-face and remotely.
Key Responsibilities- Conduct comprehensive assessments to determine eligibility for NHS Continuing Healthcare
- Provide a case management service, including reviews for eligible clients for an appropriate package of care or placement
- Chair complex MDT meetings and produce detailed written reports
We are looking for a candidate with:
- First-level qualification RGN, RMN, or RNLD Registration
- Relevant degree or extensive knowledge, skills, and experience to equivalent level
- Evidence of continuing Professional development
- Significant post-registration experience
