About the RoleThis is a challenging and rewarding role that requires strong communication and organizational skills. You will work closely with multidisciplinary teams across Health and Social Care to establish healthcare needs and support high-quality care for clients and their families in the community.
Key Responsibilities- Conduct comprehensive assessments to determine eligibility for NHS Continuing Healthcare
- Provide case management services, including reviews for eligible clients for an appropriate package of care or placement
- Chair complex MDT meetings and produce detailed written reports
- Work independently to deadlines and have excellent communication skills to engage patients and/or their representatives
To be successful in this role, you will need:
- Registration as a Registered Nurse with the NMC
- 3 years post-registration experience
- Driving license and access to a car
- IT literacy and proficiency in Microsoft Word
- High standard of English literacy skills, written and verbal
