About the Role:

We are seeking a skilled Registered Nurse to join our dialysis team in an outpatient setting. As a Nurse Practitioner - Outpatient Dialysis, you will be responsible for delivering high-quality patient care and working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities:

  • Provide direct patient care to patients undergoing hemodialysis and peritoneal dialysis
  • Conduct patient assessments, develop and implement individualized care plans, and educate patients and their families on disease management and treatment options
  • Collaborate with the healthcare team to coordinate patient care and ensure seamless transitions between departments
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Current Registered Nurse (RN) license in the state of practice
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
  • Experience in dialysis nursing or a related field preferred
