Nurse Care Practitioner

About the Role
We are seeking a skilled Nurse Care Practitioner to join our team in conducting Continuing Healthcare (CHC) reviews and assessments. As a key member of our team, you will work closely with Integrated Care Boards (ICBs) and Health Boards to provide high-quality care and support to individuals.
Main Responsibilities
As a Nurse Care Practitioner, you will be responsible for chairing Multi-Disciplinary Meetings (MDTs) and completing reviews and assessments on behalf of ICBs and Health Boards. You will work with individuals, providers, and their families to ensure that care and support plans are person-centred and meet the needs of each individual.
Key Skills and Qualifications
- Registered Nurse with a minimum of 12 months post-qualification experience
- Previous experience working within an NHS Continuing Health Team or in the community completing CHC assessments and reviews
- Ability to write clear and comprehensive reviews and assessments to a high standard
- Computer literate, specifically for Microsoft suite including Outlook & Excel
- Experience in similar work; reviewing, evaluating, and planning adult health and social care
- Good working knowledge of CHC Framework 2022
- Knowledge of Care Act 2014 and Mental Capacity Act 2005
