We are seeking a dedicated and skilled clinical practitioner to join our CHD team as a Trusted Assessor. This role involves working autonomously to assess hospital patients ready for discharge to our domiciliary care branches and care homes for reablement.

Key Responsibilities:
  • Work closely with multi-disciplinary teams to create and deliver patient pathways from hospital to home.
  • Conduct expert assessments to identify patients' health and social care needs.
  • Collaborate with discharge coordinators, community managers, and primary health support teams to ensure seamless transitions.
  • Support hospitals in preventing delayed discharges and freeing up capacity in emergency departments.
Requirements:
  • Significant experience working in social care or healthcare delivery at a supervisory level or above.
  • Substantial experience working with older peoples' services and assessing patients' needs from a hospital setting.
  • Ability to work autonomously and as part of a multi-disciplinary team.
