Job Summary
The Westbury Group Practice is seeking an experienced Advanced Nurse Practitioner to join our multidisciplinary clinical team as part of the Primary Care Network (PCN) Additional Roles Reimbursement Scheme (ARRS).
We are looking for a professional and dynamic ANP to work collaboratively with our team, utilizing their advanced clinical skills and knowledge to deliver high-quality care to patients of all ages with a wide range of undifferentiated conditions.
Main Duties of the Job
The post holder will be responsible for assessing, diagnosing, and managing patients with acute, non-acute, and chronic medical problems, working within their professional boundaries and in accordance with current guidelines and policies.
They will provide pre-arranged and same-day visits to housebound patients, demonstrating safe and effective clinical decision-making within the primary care role.
The post holder will work closely with the multidisciplinary general practice team to meet the needs of patients, building effective working relationships with external healthcare professionals and contributing to the management of the Practice's infection, prevention, and control (IPC) policies.

Job Responsibilities
- Undertake clinical nursing practice at an advanced level, utilizing knowledge and clinical skills to deliver holistic care to patients accessing primary healthcare services.
- Triage, assess, diagnose, and treat patients attending primary healthcare services with a range of acute, non-acute, and chronic medical conditions.
- Directly admit patients to secondary care in acute medical need and refer patients as appropriate to other members of the multidisciplinary team, secondary care, and other statutory and voluntary organizations.
- Undertake and interpret a range of diagnostic tests where appropriate and make clinical judgments in accordance with evidence-based practice and recognized clinical guidelines.
- Advise, support, and mentor other members of the clinical team and effectively manage own time, workload, and resources.
- Identify safeguarding concerns and make referrals as appropriate and create, implement, and manage IPC policies on behalf of the Practice.
