Sanctuary Personnel Ltd, a forward-thinking recruitment agency, is seeking a highly skilled and experienced Lead Adult Team Manager to join their Hospital Discharge Team based in Surrey.

About the Role

This permanent position offers an excellent opportunity for a motivated professional to lead, direct, and manage the service delivery of a team. As a key member of the team, you will be responsible for ensuring that all cases, including complex and high-risk situations, are progressed in line with quality, national, and legislative standards.

You will have the opportunity to work closely with colleagues and partners to develop collaborative services and partnership arrangements, promoting effective local working relationships and joint working.

Key Responsibilities
  • To lead, motivate, and develop individuals using a coaching approach to better meet current and future requirements of the service.
  • To ensure that team plans are developed to reflect wider service plans and strategies, and that the necessary resources are in place to deliver an effective and high-quality service.
  • To promote quality standards, maintain and monitor systems to collate performance information, and take appropriate actions as necessary to achieve performance targets and key performance indicators.
  • To establish effective local working relationships and joint working arrangements with partners to develop collaborative services and partnership arrangements.
  • To ensure appropriate assessment and management of risk, maintaining and implementing operational arrangements to respond to civil emergencies and preparing and updating business continuity plans to ensure continued service delivery in the event of a major incident.
  • To investigate concerns, complaints, critical incidents, and safeguarding concerns, adhering to procedures and taking the appropriate action.
Requirements

To be successful in this role, you will require:

  • A social work qualification with registration with Social Work England.
  • Experience in managing social work caseloads and supervising others.
  • Experience of community and/or group work.
  • Knowledge of adult health and social care systems, agencies, and relevant legislation.
