Role Overview

As an Outreach Keyworker, you will play a vital role in providing holistic support to female clients in Swindon. Your primary responsibility will be to assist women experiencing multiple vulnerabilities and complex needs through:

  • Needs assessment and support planning
  • Rehabilitative interventions
  • Engaging with clients through one-on-one sessions, outreach activities, and group interventions

Key Responsibilities

You will be responsible for managing a caseload of women, building strong relationships based on trust and empathy. Your role will involve:

  • Conducting thorough needs assessments to understand the unique challenges faced by each client.
  • Developing personalized support plans that address their specific needs and goals.
  • Providing practical and emotional support, guiding them towards a path of recovery and empowerment.
  • Collaborating with local organizations and stakeholders to ensure comprehensive support services are available.
