HCRG Care Group is seeking a dedicated Band 6 SCPHN School Nurse to join our team in Lancashire. As a School Nurse, you will play a crucial role in promoting health and wellbeing among children and young people.

**Key Responsibilities:**

  • The delivery of the Healthy Child Programme, including assessment of health needs, development, implementation, and evaluation of packages of care.
  • Using the Family Health Needs Assessment tool to plan and evidence care.
  • Developing individualized care plans for each child based on a needs assessment, actively involving the child/parent/carer/family in the planning and implementation of any proposed programme of care and support.

You will also contribute to the development of leadership skills among junior staff members, ensuring a smooth transition of knowledge and expertise within the team.

**Required Qualifications and Skills:**

  • SCPHN School Nurse qualification or equivalent.
  • Possession of a valid UK driving licence, with access to a car for work purposes.
  • Excellent communication and interpersonal skills.
