Job Description

The successful candidate will be responsible for carrying out comprehensive assessments and developing personalized care plans that promote independence and wellbeing. You will work closely with healthcare professionals, social workers, and community organizations to ensure seamless service delivery.

Key Responsibilities

  • Carrying out assessments and care planning in a strengths-based way.
  • Working in partnership with residents, carers, community-based organizations and other key professionals.
  • Seeking to reflect on and continually improve practice.

Requirements

  • At least 2 years of experience as a qualified Adult Social Worker.
  • Strong understanding of relevant legislation, including the Care Act and Mental Capacity Act.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
