About the Role:
This Adult Care Coordinator position offers an exciting opportunity to work within the City Adult Locality Team in Herefordshire. As a key member of our team, you will prioritize tasks, represent the Community Wellbeing Directorate, and provide supervision to allocated officers.

Responsibilities:
  1. Prioritize tasks and identify potential risks to ensure timely completion
  2. Act as a representative of the Community Wellbeing Directorate in daily interactions
  3. Supervise Assessment and Enablement Officers to enhance their skills and performance
  4. Establish and maintain legal and financial arrangements for client services
  5. Conduct regular reviews of client systems to ensure consistency and accuracy
  6. Adhere to established procedures for data collection and recording to maintain high standards
  7. Evaluate and implement council equal opportunities policies to promote inclusivity
  8. Participate in day duty and safeguarding duty as required
  9. Collaborate with nursing reviewing officers to assess health services provision


Qualifications and Experience:
  • Social Work Qualification or equivalent

Estimated Salary: £40,000 - £48,000 per annum
