Job Description:
  • Assessment & Planning:
  1. Conduct initial and ongoing assessments to determine the care needs of patients.
  2. Develop personalized care plans that address the patient's medical, physical, and emotional needs.
  3. Coordinate services across multiple providers to ensure a holistic approach to care.
Care Coordination:
  1. Liaise with doctors, nurses, specialists, and social workers to ensure the patient receives comprehensive care.
  2. Provide ongoing communication with patients and their families regarding care updates, changes, and instructions.
Monitoring & Evaluation:
  1. Regularly monitor patient progress and make adjustments to care plans as necessary.
  2. Evaluate patient outcomes and ensure care is in line with clinical guidelines and best practices.
Support & Advocacy:
  1. Act as the primary point of contact for patients and families, offering support and education on the care process.
  2. Help patients understand their treatment options, insurance coverage, and rights.
Documentation & Reporting:
  1. Maintain accurate records of patient interactions, care plans, and medical history.
  2. Ensure compliance with healthcare regulations, patient privacy laws, and organizational policies.
Collaboration:
  1. Work in collaboration with other team members, such as social workers, therapists, and physicians, to ensure high-quality patient care.
  2. Participate in team meetings to discuss patient care progress and share insights.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. We offer a competitive salary of $65,000 per annum, plus benefits and opportunities for professional growth and development.
