Clinical Lead for Virtual Ward Pharmacy Services

We are seeking a highly skilled Clinical Lead for Virtual Ward Pharmacy Services to join our team at Livewell Southwest CIC. As a key member of our multidisciplinary team, you will play a critical role in delivering high-quality clinical pharmacy services to our community.
About the RoleThe successful candidate will be responsible for leading and developing a high-quality clinical pharmacy service to Virtual Ward areas, working in partnership with system partners to maximize seamless transfers of care and achieve the best outcomes for patients. The post holder will coordinate clinical pharmacy services to the Virtual Wards, utilizing a holistic approach to support medicines optimization in various settings.
Main Duties- Leadership within the Frailty Virtual Ward regarding medicines and medicines governance
- Medicine reconciliation
- Structured medication reviews including complex polypharmacy and deprescribing
- Prescribing and accurate record-keeping regarding medications
- Answering medication queries raised by the multidisciplinary team
- Discharge facilitation
To be successful in this role, you will need to have:
- A Master's Degree or equivalent in Pharmacy
- Registration with the General Pharmaceutical Council
- Specialist knowledge acquired by postgraduate diploma or equivalent evidence of specialist knowledge, competence, and achievements in practice in pharmacy
- Evidence of Continuing Professional Development and revalidation
You will also require excellent communication and interpersonal skills, including verbal, written, and formal presentation skills for a wide variety of audiences. Computer literacy, including Microsoft Office and email, is essential. Ability to work under pressure and to tight deadlines is also required.
