About the Role

We are seeking a highly skilled and experienced Specialist Frailty Practitioner to join our team at Barts Health NHS Trust. As a key member of our Frailty Services team, you will play a vital role in delivering high-quality, patient-centered care to older adults with frailty.

Key Responsibilities
  • Deliver comprehensive geriatric assessments and develop personalized care plans for patients with frailty.
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Provide expert clinical guidance and support to colleagues to enhance patient outcomes and improve care quality.
  • Contribute to the development and implementation of evidence-based clinical guidelines and protocols.
  • Participate in quality improvement initiatives and contribute to the continuous evaluation and enhancement of Frailty Services.
Requirements
  • Registered or Chartered Health Practitioner with relevant post-registration clinical practice experience.
  • Proven experience working in the care of older people and those living with frailty.
  • Excellent interpersonal and organizational skills, with the ability to work effectively in a fast-paced environment.
  • Ability to communicate complex information to patients, families, and colleagues in a clear and compassionate manner.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
