Job Title: Inclusion Lead Practitioner

King's College Hospital NHS Foundation Trust is seeking a highly skilled and experienced professional to lead our Frailty team in delivering exceptional care to older adults.

Main Responsibilities:
  • Lead a team of Frailty Practitioners in the delivery of our 7-day service, providing day-to-day line management and support.
  • Develop and implement a work plan that focuses on clinical practice, delivering person-centred care, frailty screening, and comprehensive assessment for older adults across the Frailty Pathway.
  • Provide expert advice to older adults, their families, and other healthcare professionals.
  • Contribute to service development and transformation work across the pathway, managing data collection and evaluation.
  • Develop and implement the King's Frailty Training program.
Requirements:
  • Minimum undergraduate degree relevant to the profession.
  • Post-graduate degree relevant to the specialist area, or willingness to work towards.
  • Registered non-medical prescribing qualification.
  • Competent in using spreadsheet/data management software.
