Job Title: Frailty Care Specialist

We are seeking a highly skilled and experienced Frailty Care Specialist to join our team at Kings College Hospital. As a key member of our Frailty Service, you will play a vital role in delivering high-quality care to older adults and individuals with frailty.

Main Responsibilities:
  • Lead a team of Frailty Practitioners in the delivery of our 7-day service, providing day-to-day line management and support.
  • Develop and implement care pathways and systems of interdisciplinary documentation to improve patient outcomes.
  • Collaborate with local community, social services, and voluntary organizations to develop existing services for older adults and carers.
  • Contribute to the development of our Frailty Training program and ensure compliance with Trust policies and procedures.
Requirements:
  • Minimum undergraduate degree relevant to profession.
  • Post-graduate degree relevant to specialist area, or willingness to work towards.
  • Registered non-medical prescribing qualification.
  • Competent using spreadsheet/data management software.
