About the Role
The Department of Ageing and Health at Guy's and St Thomas' NHS Foundation Trust is seeking a highly skilled and motivated individual to join our team as a Senior Clinical Fellow in Community Frailty Transformation. This exciting opportunity will enable the successful candidate to contribute to the transformation of frailty services in the community setting.
Key Responsibilities
The post holder will work closely with the Integrated Care System in Lambeth & Southwark to develop skills in frailty care, service innovation, and integrated working. The successful candidate will be responsible for delivering Comprehensive Geriatric Assessment (CGA) and end-of-life care support for older people in the community setting.
Person Specification
The ideal candidate will have a strong background in geriatrics, general internal medicine, or primary care, with experience in community geriatrics, hospital at home, or other community-based settings. The successful candidate will also possess excellent communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team.
