Main Responsibilities:- Lead a team of Frailty Practitioners in delivering person-centred care, frailty screening, and comprehensive assessment for older people and those with frailty.
- Develop and implement care pathways, ensuring appropriate information and educational literature is available to support patients in making informed choices.
- Contribute to service development and transformation work across the pathway, managing data collection and evaluation.
- Develop and lead the King's Frailty Training program.
- Minimum undergraduate degree relevant to profession.
- Post-graduate degree relevant to specialist area, or willingness to work towards.
- Expertise in frailty care and management.
- Strong leadership and management skills.
