About the Role

We are seeking a highly skilled and experienced Care Practitioner to join our team as a Care Home Care Planning Practitioner. This is an exciting opportunity to work in a dynamic and innovative environment, contributing to the delivery of high-quality care and support to individuals in care homes.

Key Responsibilities
  • Support the delivery of enhanced MDT working in care homes across Kingston, working in partnership with care homes, Care Home (GP) Leads, acute and community services.
  • Collaborate with professionals to initiate care planning discussions, including advance care planning, comprehensive geriatric assessment, frailty, and dementia.
  • Support the implementation, management, and delivery of enhanced MDT working in care homes, including the use of Universal Care Plans.
  • Work with care home staff to identify and manage residents with complex or long-term conditions, prevent admission to hospital, and advise on nursing intervention to avoid deterioration.
  • Participate in MDT meetings to help residents navigate community and specialist services.
Requirements
  • Registered Level 1 Nurse with experience of working in care homes and community settings.
  • Experience of completing Advanced Care Plans and End of Life care to patients.
  • Knowledge of NMC Code, Advance Care Plans/Universal Care plans, and national policy governing the delivery of adults and older people's services.
  • Able to work under pressure, self-motivated, and able to motivate others.
