Adult's Learning Disabilities & Autism Advanced Practitioner Role

Ackerman Pierce Ltd is seeking an experienced Advanced Practitioner to join their Adult's Learning Disabilities & Autism team in Somerset. As a key member of the team, you will provide supervision and support for less experienced staff, support students, and take on complex cases.

Key Responsibilities:
  • Providing Case Management, Health Action Planning, and Person Centred Planning.
  • Participating in Continuing Health Care (CHC) Assessments where appropriate.
  • Participating in the Community Learning Disability Team's duty service.
Requirements:
  • HCPC Registration
  • 1-2 years post-qualifying experience within the UK Social Care industry
  • Enhanced DBS/CRB (we can obtain for you)
  • Eligibility to work in the UK.
