Job Summary

The Community Learning Disability Support Practitioner will be responsible for assessing, managing, planning, and delivering care to patients allocated to them. This includes being professionally and legally accountable for their caseload.

The post holder will work under the supervision of a Registered Nurse and continue to develop and undertake a range of delegated tasks independently.

Key responsibilities include:

  • Providing care to patients in a range of settings in the community.
  • Working autonomously to prioritize caseload, adjust working day as necessary, and make decisions without direct supervision.
  • Assisting the Community Learning Disability Nurses in identifying clients' primary and secondary health care needs and supporting them to access appropriate resources.
  • Implementing individualized health care plans in conjunction with the Community Learning Disability Nurses.
  • Assisting in the assessment, planning, implementation, and evaluation of health plans within a multi-disciplinary framework.

The successful candidate will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies.

Requirements
  • NVQ2/3 in Health & Social Care
  • Foundation Degree for Assistant Practitioner in nursing
  • Experience in working in Learning Disability setting for at least 2 years (either in-patient or community setting)
  • Experience of working as part of a multi-disciplinary team under appropriate supervision
  • Experience of completing Hospital Passport and health action plan by liaising with the families, GPs, and carers
What We Offer

NELFT North East London Foundation Trust offers a competitive salary of £26,530 to £29,114 per annum, plus High Cost Area Supplement. The successful candidate will also have access to ongoing training and development opportunities.
