Job Description

We are seeking a highly skilled Advanced Clinical Practitioner to join our team in the North West Boroughs Healthcare NHS Foundation Trust. This is an exciting opportunity to work with clients who pose significant challenges and risk to themselves or others, with associated problems such as mental health issues, complex communication needs, profound and multiple disabilities, long-term conditions, and forensic behaviour.

Main Responsibilities:
  • Provide expert clinical practice to clients with complex challenging behaviour and/or forensic behaviours who may be at risk of placement breakdown and/or hospital admission.
  • Take the lead role in facilitating/co-ordinating access to primary and secondary health care services for people with learning disabilities.
  • Develop and influence changes at both local and trust-wide levels.
  • Responsible for the assessment of need, care planning and care management of clients and carers in accordance with the Trust Effective Care Co-ordination policy within Mental Health Services.
About the Role:

This post requires a highly motivated and experienced professional to work alongside the established Community Learning Disability Team, Clinical Team Managers, and associated specialists to develop, initiate, lead, and manage clinical assessment, staff training, and special projects consistent with the aims of the Trust, service plan, and service operational policy.

Requirements:
  • A postgraduate qualification, i.e., masters or equivalent level of training through experience.
  • Evidence of post-basic education to degree level or evidence of performance to an equivalent level.
  • Evidence of post-basic education in Positive Behavioural Support degree level.
  • Experience of working with clients with learning disabilities/mental health issues.
Salary and Benefits:
