Main Responsibilities:
  • Lead a team of Frailty Practitioners in delivering person-centred care, frailty screening, and comprehensive assessment for older people and those with frailty.
  • Develop and implement care pathways, ensuring appropriate information and educational literature is available to support patients in making informed choices.
  • Contribute to service development and transformation work across the pathway, managing data collection and evaluation.
  • Develop and lead the King's Frailty Training program.
Key Requirements:
  • Minimum undergraduate degree relevant to profession.
  • Post-graduate degree relevant to specialist area, or willingness to work towards.
  • Expertise in frailty care and management.
  • Strong leadership and management skills.
