About the Role

Key Responsibilities
  • Manage a defined caseload of patients, including ongoing assessment of care needs and development of care programs.
  • Deliver high-quality, evidence-based care to patients, their families, and carers within the community.
  • Maintain detailed, accurate, and confidential patient records, adhering to Trust policies.
  • Oversee the activities of your team, promoting high standards of care and best practices.
Requirements
  • Registered Nurse - Adult qualification with NMC registration.
  • Active on the Mentor Register.
  • Evidence of leadership and management development.
  • Recognized coaching qualification or willingness to undertake.
  • Non-Medical Prescriber/Nurse Prescriber Qualification.
