Job Summary

We are seeking an experienced Trauma Rehabilitation Specialist to join our team at University Hospitals Sussex NHS Foundation Trust. As a key member of the multidisciplinary team, you will be responsible for coordinating the rehabilitation pathway for patients with complex needs.


About the Team


  • Main Responsibilities:
  • Develop and maintain a database/information system to demonstrate timing and coordination of rehabilitation decisions, referrals, and transfer of care.
  • Take responsibility for ensuring that appropriate training and development needs are identified and implemented to support these activities.
  • Maintain accurate and comprehensive daily patient records, including any variances from planned activity and maintaining MDT communication regarding the status of rehabilitation referrals.
