About the Role

We are seeking a skilled Mental Health Nurse to join our team at Livewell Southwest, an independent social enterprise delivering integrated health and social care services across Plymouth, South Hams, and West Devon. As a Mental Health Recovery Specialist, you will be responsible for providing individualized care to service users, assessing their needs, developing personalized care plans, and working collaboratively with multi-disciplinary teams.

Key Responsibilities
  • Deliver high-quality, person-centered care to service users, incorporating assessment, planning, implementation, review, and evaluation of care plans
  • Act as a key worker for identified groups of clients, maintaining accurate records and communicating effectively with colleagues and stakeholders
  • Contribute to multi-disciplinary teams, leading handovers and meetings, assessing new referrals, and attending clinical reviews
  • Provide verbal prompting and additional support to ensure service users' basic physical health and personal care needs are met
Requirements

To be successful in this role, you will need:

  • A valid RMN qualification and registration with the NMC
  • Experience in multi-disciplinary working and relevant transferable healthcare experience
  • Competency in leadership skills, including supervisory roles

Salary

The salary for this position is £32,000 - £45,000 per annum, depending on experience.
