Job Summary

Livewell Southwest is seeking a skilled Discharge Support Worker to join our team in providing mental health services. As a key member of our multidisciplinary team, you will work closely with individuals, their relatives, and carers to promote recovery and facilitate safe discharge from hospital.

Main Responsibilities

  • Assist the multidisciplinary team in delivering physical, social, and psychological care to individuals with mental health illness.
  • Provide support to an allocated group of individuals with more complex needs, promoting their recovery and maintaining them in their community environment.
  • Assist the multidisciplinary team to assess, plan, implement, and evaluate care, and implement discharge plans.
  • Deliver physical, social, and psychological care to individuals with mental health difficulties, working in hospital and community settings.
  • Liaise with relatives, carers, and other agencies regarding discharge plans.
