Job Summary
Bridgewater Community Healthcare NHS Foundation Trust is recruiting a Clinical Nurse Associate to join its team. As a Clinical Nurse Associate, you will play a vital role in delivering high-quality care to patients in their homes. This is an exciting opportunity for an enthusiastic and motivated individual to develop their skills and knowledge in a dynamic and evolving healthcare environment.
Main Duties of the Job
The Clinical Nurse Associate will assist in identifying health needs and participate in the delivery and evaluation of care interventions. This will involve working closely with a registered practitioner to develop and implement individual care plans. The postholder will also contribute to the development and maintenance of documentation and IT systems.

Key Responsibilities
The successful candidate will be responsible for:
- Functioning as an effective member of the District Nursing and wider multi-disciplinary team
- Maintaining clear, concise, and legible documentation and adhering to prescribed standards and protocols
- Using IT systems for relevant tasks, accessing patient records and results, requesting investigations, and data collection
- Obtaining consent in accordance with Trust Consent Policy
- Using safe handling techniques and promoting patient mobility, utilizing mobility aids and equipment where appropriate
- Maintaining a patient-orientated approach towards care delivery and rehabilitation
- Undertaking designated patient assessments according to protocol
- Planning and delivering high standards of care in conjunction with relevant professionals where appropriate
- Implementing and evaluating individual care plans, utilizing relevant outcome measures
- Recognizing the need for referral and undertaking appropriate referrals to specialist nurses/practitioners/services within the NHS, independent, and voluntary sectors
- Promoting and providing health education specific to intermediate care and in line with national and local policies
- Undertaking, monitoring, and recording basic observations, including temperature, blood pressure, pulse, respiration, peak flow, and blood glucose levels
- Utilizing clinical skills appropriately to enhance the delivery of patient care, including venepuncture and removal of clips/sutures
- Implementing and evaluating a plan of care for simple wound management, as directed by a registered practitioner
- Monitoring and caring for patients with respiratory conditions, including the use of oxygen therapy and positioning to aid expectation/breathing
Person Specification
The ideal candidate will have:
- Competencies achieved within the workplace and attendance at a Higher Education Institute (HEI), as part of the Trainee Nurse Assistant programme
- Understanding of the limits of the Nursing Associate role and responsibilities
- Oral and written communication skills
- Computer literacy
- Ability to work collaboratively across teams, agencies, and organisational boundaries
- Nurse Associate registration
