Job Description

You will provide early rehabilitation to trauma, surgical, medical, and neurological patients, as well as manage long-term life-limiting conditions and conduct complex discharge planning. A key aspect of this role is collaborating with colleagues to ensure seamless transitions between departments and optimizing patient flow.
