Job Summary:

The West London NHS Trust is seeking a skilled Healthcare Programme Manager to lead the Healthy Ealing programme. This initiative aims to enhance health and social care outcomes for the residents of Ealing. The successful candidate will drive the programme's objectives, ensuring effective delivery and meeting the needs of the community.

Key Responsibilities:

  • Coordinate a wide cross-section of stakeholders to run health outreach events for local targeted population groups.
  • Analyse and interpret national and local data to develop targeted local projects.
  • Assist local partners in assessing health and social care variation, system challenges, and capacity needs.
  • Mobilise and provide education to local partners around social determinants of health and health equity.
  • Organise, conduct, and facilitate improvement planning in line with health needs assessments.
  • Engage diverse community partners in developing, implementing, and evaluating policies, programs, and services.
