Community Mental Health Nurse

Job Overview
We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team in Luton. As a Band 6 Community Mental Health Nurse, you will work as an autonomous practitioner within a dedicated Multidisciplinary Hub.
Main Duties
You will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the Operational Policy. This will involve the assessment, care planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the clients.
Key Responsibilities
- Assess, plan, implement and evaluate care for clients with mental health needs
- Liaise with agencies involved with clients to ensure comprehensive care
- Work within the principles of the agreed model of care for the service
- Participate in Quality Improvement (QI) projects within the Team
Person Specification
Essential Criteria
- Registered Nurse (Mental/ Learning Disability Nurse)
Desirable Criteria
- Experience of working with people with mental health problems/learning disabilities within acute/ forensic setting
- Managing challenging and aggressive behavior
- Working within a multidisciplinary team environment
- Experience of working in a multi-cultural environment and knowledge of methods to develop culturally sensitive practice
Ability and Skills
Essential Criteria
- Good verbal and written communication skills
- Ability to assess, plan, implement and evaluate care
- Understanding of de-escalation in the context of acute or Forensic nursing
- Able to work systematically and objectively
