About the Role

We are seeking a skilled Community Practitioner to join our Early Intervention in Psychosis team in Gateshead. As a key member of our multidisciplinary team, you will work closely with service users, their families, and carers to provide high-quality, recovery-focused care.

Key Responsibilities
  • Work with service users to develop and implement holistic care plans, focusing on recovery and wellbeing.
  • Deliver evidence-based therapeutic interventions, including structured clinical management and cognitive-behavioral therapy.
  • Conduct comprehensive risk assessments and develop risk management plans using the Biopsychosocial Risk Assessment Tool.
  • Participate in duty rota and triage new referrals, ensuring timely and effective support for service users.
  • Support health promotion and education, promoting a culture of learning and professional development within the team.
Requirements
  • Professional qualification and current registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions and skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Mentor training and up-to-date knowledge/experience of relevant legislation, including CPA, Safeguarding, MAPPA, and Child Protection.
