We are seeking a highly skilled Advanced Nurse Practitioner to join our Primary Care Network in Newcastle. As an ANP, you will be responsible for providing high-quality care to patients, conducting comprehensive assessments, and developing treatment plans.

Key Responsibilities:

  • Autonomous Consultations: Provide independent care to patients, handling a variety of clinical cases.
  • Patient Assessments: Conduct thorough assessments for adults and children, diagnosing and managing conditions effectively.
  • Treatment Plans: Develop and implement treatment plans, ensuring the best outcomes for patient care.
  • Referral Management: Make referrals to specialists or other healthcare professionals when necessary.
  • Multi-Site Coverage: Provide clinical support across five local practices, ensuring continuity of care for the community.
  • Collaborative Working: Liaise with GPs, nurses, and other healthcare professionals to maintain a cohesive team-based approach to care.
  • Record Keeping: Maintain accurate and up-to-date patient records using EMIS.

Requirements:

  • Must be able to work independently with minimal supervision.
  • Experience in seeing both adults and children is essential.
  • Strong clinical skills and decision-making ability.
  • EMIS System Experience
  • ANP Experience within a similar setting
