About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team in providing high-quality patient care in a primary care setting.

Key Responsibilities
  • Provide acute same-day and planned care to patients registered at the surgery, including telephone triage and face-to-face assessments.
  • Refer patients to other medical specialties for assessment as needed.
  • Undertake annual reviews of patients' understanding and ability to self-manage, as well as medication reviews within scope of competency.
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to their health.
  • Provide advanced, specialist nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
  • Undertake the collection of pathological specimens and request pathology/radiology services as necessary.
  • Process and interpret pathology and other test results as required.
  • Provide chronic disease clinics, delivering patient care as necessary, and referring patients to secondary/specialist care as required.
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Carry out home visits to patients in our community.
  • Prioritize health issues and intervene appropriately.
  • Recognize, assess, and refer patients presenting with mental health needs.
  • Support patients in the use of their prescribed medicines or over-the-counter medicines, reviewing annually as required.
  • Support the clinical team with all safeguarding matters in accordance with local and national policies.
  • Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately.
  • Deliver opportunistic health promotion where appropriate.

Requirements
  • Master's degree in Advanced Practice or equivalent post-graduate qualification.
  • Registered Nurse Practitioner/Advanced Nurse Practitioner with NMC.
  • Full UK driving license.
  • General practice experience.
  • Advanced clinical skills in a primary care setting with proficiency in using clinical diagnostic tools and conducting complex clinical procedures.
