About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team in providing high-quality patient care in a primary care setting.

Key Responsibilities
  • Provide acute same-day and planned care to patients registered at the surgery, including telephone triage and face-to-face assessments and home visits.
  • Refer patients to other medical specialties for assessment if deemed appropriate.
  • Undertake annual reviews of patients' understanding and ability to self-manage and medication reviews within scope of competency.
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patients' health.
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF.

Person Specification
  • Master's degree in Advanced Practice or equivalent post-graduate qualification.
  • Registered Nurse Practitioner/Advanced Nurse Practitioner with NMC.
  • Full UK Driving Licence.
  • General Practice Experience.
  • Advanced clinical skills in a primary care setting with proficiency in using clinical diagnostic tools and conducting complex clinical procedures.
