Job Title: Adult Advanced Practitioner

Service Care Solutions Ltd is seeking a skilled and experienced Adult Advanced Practitioner to join our team in an Urgent Care Centre across the West Midlands.

About the Role:

We are looking for a highly motivated and autonomous practitioner to work within our multi-disciplinary team, delivering evidence-based nursing services to patients. As a Nurse Prescriber, you will be responsible for a number of clinical areas, including health promotion, chronic disease management, health prevention, and well-man clinics. You will also support the management team in reviewing clinical policy and procedure.

Key Responsibilities:
  • Deliver evidence-based nursing services to patients
  • Work autonomously as part of the multi-disciplinary team
  • Prescribe medication in accordance with PGD and within own scope of practice
  • Review medication for effectiveness, need, and national guidance
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions
  • Support the clinical team in developing individual treatment plans for chronic disease patients
  • Identify, manage, and support patients at risk of developing long-term conditions

Requirements:

To be successful in this role, you will need to have:

  • Experience as a Nurse Prescriber
  • Knowledge of health promotion, chronic disease management, and health prevention
  • Ability to work autonomously as part of a multi-disciplinary team
  • Strong communication and interpersonal skills
