Job Title: Advanced Clinical Practitioner

Service Care Solutions Ltd is seeking an experienced Advanced Clinical Practitioner to join our team in an Urgent Care Centre across the West Midlands.

About the Role:

We are looking for a skilled and autonomous practitioner to work within our multi-disciplinary team, delivering evidence-based nursing services to patients. As a Nurse Prescriber, you will be responsible for health promotion, chronic disease management, health prevention, and well-man clinics, as well as supporting the management team in reviewing clinical policy and procedure.

Key Responsibilities:
  • Deliver high-quality nursing services to patients, working autonomously and as part of the service multi-disciplinary team.
  • Prescribe medication in accordance with PGD and within own scope of practice.
  • Review medication for effectiveness, need, and national guidance.
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in developing individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
Benefits:

We offer a competitive hourly rate, with Monday to Friday rates at £45 per hour, Saturday/Sunday rates at £47 per hour, and night rates at £49 per hour. Additionally, we provide a £250 sign-up bonus for new nurses, a £250 referral fee bonus for successful referrals, and a range of benefits including DBS disclosures, 3 weekly payroll runs, and a £250 training allowance.
