Job Title: Adult Advanced Practitioner

Are you a skilled and experienced nurse looking for a challenging and rewarding role in an Urgent Care Centre? We have an exciting opportunity for an Advanced Clinical Practitioner to join our team in the West Midlands.

Job Summary:

We are seeking a highly motivated and skilled nurse to work as part of our multi-disciplinary team in an Urgent Care Centre. As an Advanced Clinical Practitioner, you will be responsible for delivering evidence-based nursing services, working autonomously as a Nurse Prescriber, and supporting the management team in reviewing clinical policy and procedure.

Key Responsibilities:

  • Deliver high-quality nursing care to patients in accordance with clinical-based evidence, NICE, and the NSF.
  • Work as part of the service multi-disciplinary team to deliver care within your scope of practice to the entitled patient population.
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in developing individual treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions.
  • Provide routine nursing care to patients as required.
