Job Title: Adult Advanced Practitioner

We are seeking an experienced Advanced Nurse/Clinical Practitioner to join our team at an Urgent Care Centre in the West Midlands.

Key Responsibilities:

  • Deliver evidence-based nursing services as part of the multi-disciplinary team.
  • Work autonomously, prescribing medication as needed, and reviewing treatment plans for acute and non-acute conditions.
  • Develop and implement health promotion and wellbeing programmes, and support the clinical team in developing individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.

Requirements:

  • Experience as an Advanced Nurse/Clinical Practitioner in an Urgent Care Centre or similar setting.
  • Ability to prescribe medication in accordance with PGD and within your own scope of practice.
  • Strong communication and interpersonal skills, with the ability to work effectively as part of a team.
  • Knowledge of health promotion and wellbeing principles, and experience in developing and implementing programmes.
