Job Title: Advanced Clinical Practitioner

Service Care Solutions Ltd is seeking an experienced Advanced Clinical Practitioner to join our team in an Urgent Care Centre across the West Midlands.

About the Role:

We are looking for a skilled and autonomous practitioner to deliver evidence-based nursing services, working as part of a multi-disciplinary team. The successful candidate will be responsible for delivering care within their scope of practice to the entitled patient population.

Key Responsibilities:
  • Deliver evidence-based nursing services, working autonomously as part of a multi-disciplinary team.
  • Responsible for the delivery of health promotion, chronic disease management, health prevention, and wellbeing programmes.
  • Support the management team in reviewing clinical policy and procedure.
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in developing individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions.
  • Provide routine nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
  • Prescribe medication in accordance with PGD and within own scope of practice.
