Clinical Care Specialist for Community and Care Home Settings

About the Role
We are seeking a skilled Clinical Care Specialist to join our team in delivering high-quality care to patients in various settings across Sandwell and West Birmingham. As an Advanced Nurse Practitioner, you will work closely with our multidisciplinary team to provide comprehensive care to our patients.

Key Responsibilities
- Deliver care in a variety of settings, including patients' homes, care homes, and community hubs.
- Work collaboratively with our team to provide comprehensive care to our patients.
- Develop and implement care plans that meet the individual needs of our patients.
Qualifications
- RN with Current NMC registration.
- Postgraduate qualification relevant to clinical specialty.
