Sirona care & health is excited to announce the establishment of two new Community Transition Hubs aimed at enhancing patient discharge planning from the point of admission to their eventual return home. These hubs will integrate efforts from various stakeholders, including acute trusts, community partners, local authorities, and voluntary sector organizations, to form a cohesive multidisciplinary team.
The primary objective is to collaborate effectively to ensure patients achieve a 'Home First' approach by assembling a diverse team of professionals to facilitate seamless discharges.
We are looking for candidates with experience in community settings and a solid understanding of the clinical acuity levels that can be managed in home environments. Additionally, familiarity with rehabilitation pathways, discharge planning, and mental capacity considerations, along with excellent interpersonal and negotiation skills, is essential.
Key Responsibilities
As a vital member of the multidisciplinary team, you will:
- Conduct comprehensive, person-centered assessments to enable smooth transitions from acute care to appropriate discharge pathways.
- Manage and prioritize referrals received by the Community Transition Hub, collaborating with patients, families, and hospital staff to determine the best discharge options.
- Engage with colleagues across the hub to discuss complex cases and formulate joint action and discharge plans.
- Utilize multidisciplinary assessment techniques and sound clinical judgment to ensure timely hospital discharges to suitable environments.
- Effectively manage a diverse caseload, adapting your daily schedule in response to system-wide demands and optimizing community resources.
- Employ high-level problem-solving skills, gathering insights from various sources and leveraging professional expertise to guide decision-making, including conducting mental capacity assessments and making best interest determinations as needed.
