About Adult Social Care
">We are seeking a dedicated Disability Support Professional to join our All Age Disability Team. As a key member of our team, you will play a vital role in providing support services for young people with Special Educational Needs and Disabilities (SEND).
">Job Summary:
">The successful candidate will be responsible for supporting service delivery, providing personal care, and maintaining accurate records. You will also assist with facilitating sessions and have experience of working with young people with SEND needs or vulnerable adults.
">Key Responsibilities:

- Deliver high-quality support services to young people with SEND needs, promoting their independence, dignity, choice, and control.">
- Provide personal care and manual handling duties as required, ensuring the health and wellbeing of service users.">
- Assist with planning and facilitating sessions in line with individual EHCP outcomes.">
- Maintain accurate and up-to-date records in electronic and paper systems.">
Requirements:
">- ">
- Holding a Care Certificate or willingness to undertake.">
- Working knowledge of alternative communication systems.">
- Experience of working with young people with SEND needs or vulnerable adults, or within a PBS service.">
- Ability to work collaboratively as part of a team.">
- Excellent communication and interpersonal skills.">
Salary and Benefits
">We offer a competitive salary of £25,100 - £28,000 per annum, depending on experience, plus a range of benefits, including opportunities for professional development and a supportive team environment.
