Job Summary:

We are seeking a highly skilled and compassionate Healthcare Professional - Complex Care to join our team. As a key member of our team, you will be responsible for delivering high-quality care to individuals with complex needs, and developing and implementing effective care plans to meet their unique needs.

Key Responsibilities:

  • Deliver high-quality care to individuals with complex needs.
  • Develop and implement effective care plans to meet the unique needs of each individual.
  • Maintain high standards of care, adhering to relevant regulations and guidelines.
  • Manage complex care packages, including risk assessments and care planning.

Requirements:

  • Prior experience in complex care, ideally within a home care setting.
  • Level 3 in Health and Social Care or equivalent.
  • Clinical expertise in complex care needs, such as tracheostomy, ventilation, bowel care, and PEG feeding.
  • Excellent leadership and communication skills.
  • A full UK driving license and access to your own vehicle.

Salary and Benefits:
