About the Role
We are seeking a Health Visitor to join our team at Stockport NHS Foundation Trust. As a Health Visitor, you will play a key role in improving the outcomes for children and families in Stockport.
The Health Visiting Service in Stockport is a key part of our integrated Start Well Team, working closely with early years providers, GPs, Midwives, and Stockport Family colleagues to deliver the Healthy Child Programme and early years pathways.
Main Responsibilities
As a Health Visitor, you will lead the Healthy Child Programme 0-5 and manage a named health visitor mixed caseload of universal, enhanced, and specialised pathway families.
You will provide holistic assessment using the My World Triangle and utilise planning skills to identify and address health needs in a defined local population.
You will provide early help support to families when required, utilising the Early Help Assessment and within a Team Around the Early Years model.
