About the Role

We are seeking a skilled and dedicated Health Visitor to join our team at the Stockport NHS Foundation Trust. As a key member of our Start Well Team, you will play a vital role in improving health outcomes for children and families in Stockport.

Your primary responsibilities will include leading the Healthy Child Programme 0-5 and managing a named caseload of universal, enhanced, and specialized pathway families. You will work closely with other healthcare professionals to provide holistic assessments, develop care plans, and deliver early help support to families as needed.

Key Requirements
  • Registered Health Visitor
  • Broad based experience in nursing and health visiting
  • Excellent communication and interpersonal skills

In addition to your clinical expertise, we value your ability to be adaptable, reliable, and approachable. You will also need to demonstrate up-to-date knowledge of national agendas affecting children and families.
