This role is an exciting opportunity to make a positive difference in people's lives as a Mental Health Practitioner at Derbyshire Healthcare NHS Foundation Trust. You will be working in a dynamic environment, collaborating with various stakeholders to provide expert mental health advice and support.
The key duties of this position include:
- Conducting comprehensive mental health assessments and identifying individual needs
- Developing and implementing effective care pathways for individuals with complex needs
- Maintaining strong relationships with referral agencies, ensuring seamless communication and collaboration
- Supporting individuals in accessing necessary community resources and services
- Collaborating with multidisciplinary teams to ensure holistic care approaches
