Key Responsibilities:

  • Develop and maintain strong relationships with local partners, ensuring seamless coordination of services.
  • Conduct home visits and assessments to identify areas of need and develop personalized support plans.
  • Collaborate with colleagues to design and deliver effective interventions that promote positive outcomes.

What You'll Need:

  • Significant experience of working with vulnerable populations, preferably in a social care setting.
  • Strong communication and interpersonal skills, with the ability to build trust with clients from diverse backgrounds.
  • A DBS check will be required prior to commencement.

Working with Us:

  • 37 hours per week, full-time.
  • Fixed-term contract until 31 March 2025.

Location: Bridgend County Borough Council. Salary Range: £27,711 - £28,624 per annum.
