We are seeking a skilled Clinical Pharmacist to join our team of healthcare professionals working remotely across the UK. As a clinical pharmacist, you will play a vital role in supporting and working alongside a team of pharmacists and GPs.

Key Responsibilities:
  • Conduct medication reviews and reconciliation post-discharge
  • Deliver clinical leadership on medicines optimisation and quality improvement
  • An understanding of QOF, CQC and local incentive scheme workstream
  • Create and follow clinical pharmacy programme according to practice policy
  • Conduct long term condition reviews

The successful candidate will have a minimum of 12 months experience of working within general practice/primary care or be willing to undergo training. Experience with EMIS or System 1 is desirable but not essential as training can be provided.

As a remote Clinical Pharmacist, you will be part of a network of over 200 pharmacists nationally and have dedicated senior pharmacist support available.
